Insurance Participation
We are pleased to participate with several major insurance plans. Please ask our business office for current coverage information with your insurance carrier.  

Billing Policies
Payment is expected at the time of each office visit unless other arrangements have been made. Insurance co-pays are expected to be made at the time of your visit. We’ll gladly file all insurance claims for you for reimbursement. Please bring your current insurance information with you to your appointment. Cash, checks and major credit cards accepted. Emergency Care
A physician from our office is on call 24 hours a day and can be reached by calling our main office number, (616) 459-4131. When calling after normal office hours, an answering service will make contact with the on-call physician, who will respond to your needs as quickly as possible. Requests for prescriptions and renewals should be made during normal office hours. In the event of a severe emergency, go immediately to a hospital emergency room and request that we be contacted.
