Financing Options
At Elite, we work with a variety of insurance companies for our patients requiring procedures covered by insurance. Including but not limited to Medicare, Priority Health, Blue Care Network, Blue Cross Blue Shield and others.
A $500 deposit is required at the time you are added to your doctor’s surgical schedule for cosmetic procedures. The remaining balance is required 10 business days prior to surgery. If, for any reason, you have to cancel surgery, your non-refundable deposit will be credited to your surgical fee if your rescheduled surgery is performed within 12 months of your original surgical date.

Billing Policies
Payment is expected at the time of each office visit unless other arrangements have been made. Insurance co-pays are expected to be made at the time of your visit. We’ll gladly file all insurance claims for you for reimbursement. Please bring your current insurance information with you to your appointment. Cash, checks and major credit cards accepted. Post-Op Instructions
Emergency Care
A physician from our office is on call 24 hours a day and can be reached by calling our main office number, (616) 459-4131. When calling after normal office hours, an answering service will make contact with the on-call physician, who will respond to your needs as quickly as possible. Requests for prescriptions and renewals should be made during normal office hours. In the event of a severe emergency, go immediately to a hospital emergency room and request that we be contacted.
